چکیده

Abstract INTRODUCTION The onco-metabolite, 2-hydroxyglutarate (2HG), is non-invasive biomarker detected by magnetic resonance spectroscopy (MRS), indicating isocitrate dehydrogenase (IDH) mutant glioma. Especially in patients with brainstem lesions where biopsies are at high risk of causing neurological deficit, 2HG-MRS may be useful. Here, we examined the utility for diagnosing IDH adult diffuse glioma and ability prognosis prediction. METHODS Three tesla -MRS (3T-MRS) was performed on radiographically confirmed tumor. A single-voxel set lesion reference to T2 / FLAIR image, analyzed according 2HG-tailored MRS protocol (Philips, Achieva, PRESS, TE 35 ms). All underwent biopsy using an intraoperative navigation system (Brain LABTM) or stereotactic (Komai’s CT-stereotactic frame) before initial treatment. H3K27M mutation status were immunohistochemistry DNA sequence. We relationship between 2HG values patient survival period. RESULTS Ten (7 male 3 female, median age 33.5) biopsy. Four had 4 (1 R132H, 2 R132S, 1 R132G). Two neither nor mutation. H3-K27 mutations mutually exclusive. tumor located pons mainly. There no significant radiological difference conventional MRI Pearson's chi-square test demonstrated that concentrations > 1.5 mM 100% sensitivity 83.3% specificity (p = 0.0098). overall 130.5 months (n=4) 25 wild-type (n=6), respectively. CONCLUSIONS 3T-MRS able detect noninvasive study helpful diagnosis predicting patient.

Detection of 2-hydroxyglutarate in IDH-mutated glioma patients by in vivo spectral-editing and 2D correlation magnetic resonance spectroscopy.

Mutations in the gene isocitrate dehydrogenase 1 (IDH1) are present in up to 86% of grade II and III gliomas and secondary glioblastoma. Arginine 132 (R132) mutations in the enzyme IDH1 result in excess production of the metabolite 2-hydroxyglutarate (2HG), which could be used as a biomarker for this subset of gliomas. Here, we use optimized in vivo spectral-editing and two-dimensional (2D) cor...

Urine 2-Hydroxyglutarate in Glioma

We congratulate Fathi et al. [1] for their effort to analyze the oncometabolite 2-hydroxyglutarate (2-HG).This is an interesting work evaluating 2-HG in serum, urine, and cerebrospinal fluid (CSF) from patients with isocitrate dehydrogenase (IDH) mutant and IDH wild-type (WT) glioma. The objective of this study was to use 2-HG as a surrogate biomarker for IDH mutation. Serum 2-HGwas found to be...

Magnetic Resonance Spectroscopy for Detection of 2-Hydroxyglutarate as a Biomarker for IDH Mutation in Gliomas

Mutations in the isocitrate dehydrogenase (IDH)1/2 genes are highly prevalent in gliomas and have been suggested to play an important role in the development and progression of the disease. Tumours harbouring these mutations exhibit a significant alteration in their metabolism resulting in the aberrant accumulation of the oncometabolite 2-hydroxygluarate (2-HG). As well as being suggested to pl...

2-hydroxyglutarate detection by magnetic resonance spectroscopy in subjects with IDH-mutated gliomas

Mutations in isocitrate dehydrogenases 1 and 2 (IDH1 and IDH2) have been shown to be present in most World Health Organization grade 2 and grade 3 gliomas in adults. These mutations are associated with the accumulation of 2-hydroxyglutarate (2HG) in the tumor. Here we report the noninvasive detection of 2HG by proton magnetic resonance spectroscopy (MRS). We developed and optimized the pulse se...
